Medical Data Consent
By submitting medical materials, you voluntarily permit their use for a preliminary review and care coordination.

1. Data covered
Scans, X-rays, photographs, reports, medical history, diagnoses, previous treatment, medicines, allergies and contraindications that you choose to provide.
2. Purpose
To obtain preliminary opinions from independent partner-clinic doctors, compare possible approaches, prepare an indicative estimate and coordinate the care pathway. This does not replace an in-person diagnosis.
3. Sharing
Materials may be shared with an authorised Odenta coordinator, a secure storage provider and only those partner clinics or doctors needed for your request. They are not published or used in advertising without separate consent.
4. Voluntary submission
Medical file submission is voluntary. You may start without files and ask the coordinator what is needed. Without sufficient materials, a preliminary review may be less specific.
5. Withdrawal and minors
Consent lasts until the purpose is met or it is withdrawn through odentauz@outlook.com, +998 90 217 73 37. New sharing then stops and data is deleted or anonymised unless lawfully retained. A legal representative must consent for a patient under 18.
